Gamechanger: Shifting from tobacco control to ending the industry's influence for good

The concept of “Endgame” is a strategy to move to a time when commercial tobacco and nicotine – with the exception of FDA-approved medications – are no longer used. This tobacco-free future can be achieved through a series of policies to decrease the access, the appeal, and the addictiveness of products by increasing cessation interventions and services and de-normalizing tobacco use, ultimately relegating it to a behavior of the past. While we are at the precipice of action nationally, this strategy is already being tackled at the local level and internationally, with actions that can bring tobacco and nicotine addiction – and all the harms they cause – to a natural end in our society. By implementing flavor restrictions at all levels, restricting points of sale for tobacco products, reducing nicotine levels in cigarettes and other products, and improving access to current and new cessation interventions, we can and will be firmly on the road to a tobacco-free future.

This movement is gaining increasing momentum among public health experts, policymakers and the general public. More than 120 localities have prohibited all flavored tobacco sales within their borders, and several states including California, Minnesota and Massachusetts have moved to adopt endgame strategies. A 2021 survey conducted by the Centers for Disease Control and Prevention found that 62.3 percent of adults supported a policy prohibiting the sale of menthol cigarettes, and more than 57 percent of adults supported a policy prohibiting the sale of all tobacco products.

This Truth Initiative Impact Series will discuss the potential of the endgame movement, recent domestic and global successes, and how to advance future efforts. The panel will explore how to move to end the influence of tobacco, rather than controlling it, and how to can take bolder steps that will more quickly advance society toward the end of tobacco and ultimately nicotine addiction.
